Polk HealthCare Plan Increases Income Eligibility Levels to Assist More People

The income eligibility guidelines for the Polk HealthCare Plan (PHP) are expanding to assist more residents in need in response to the COVID-19 pandemic. Those who applied previously and were turned down due to income reasons are encouraged to apply again.

Qualifying maximum monthly household income ranges from $1,481 for one person to $3,048 monthly for a household of four.

Other eligibility requirements include:

• Must be a current, full-time resident of Polk County

• No health insurance

• Not eligible for Medicaid, Medicare or other health insurance

• Must be either a citizen of the U.S. or a lawfully admitted immigrant and have a Social Security card

To apply, residents can fill out an electronic application (and upload the supporting documentation needed to confirm eligibility) or schedule an appointment with a case manager to apply for the PHP and other health care services.

The PHP is for those caught in the gap between not qualifying for Medicaid or Medicare, and not being able to afford marketplace insurance. While the PHP is not insurance, it is a way for qualifying individuals to get healthcare. The provider network includes primary care and specialty care physicians, urgent care centers and five area hospitals. Sponsored services include primary and specialty care services, routine diagnostic testing, lab, radiology, hospital inpatient, outpatient and emergency services.
